The admissions office is located on the ground floor of the hospital alongside the elevator. You are required to complete the relevant paperwork prior to admission and may also opt to enter the hospital as a private patient with no out of pocket expenses. By opting to admit privately, you will be assisting the hospital financially helping us to purchase further equipment and offer additional services .
Staff will advise you on any queries you may have regarding admission procedures. You will receive relevant information about any conditions applying to your surgery or otherwise prior to admission.
On the day of your admission:
It is important that you arrive punctually for your procedure to allow time for preparation. If you think you will be late, please telephone the hospital on (03) 5150 3333.
You may be required to bring any of the following items with you:
Current medicines and a list of medications that you have taken in the last month
X-rays, ultrasounds or reports where relevant
Referral letters
Medicare card, private health insurance details, pension card, Department of Veterans Affairs card, TAC and WorkCover claim numbers including employer details
Pharmaceutical entitlement number
Two telephone contact numbers of a friend or a relative
A labelled overnight bag to store your clothes
Name, address and telephone number of your local doctor
Cotton nightie or pyjamas
If you have appointed a Medical Enduring Power of Attorney under the Medical Treatment Act 1988 (Vic) (Section 5A) please bring the original of the power of attorney with you and a copy will be taken and filed in your medical history.
